Rates & Out-Of-Network Benefits

Rates. Comprehensive evaluations at DHW ranges from $2000 – $4000.

Payments. Payment is due at the time of service. If needed, a payment plan option can be created. Payments may be made by credit card, debit card, FSA, or HSA.

Insurance. We are out-of-network with all insurance companies. All clients are directly responsible for payment of services. If your insurance does allow out-of-network services, we can provide a superbill for you to submit to your insurance company for reimbursement.

How do Out-of-Network Benefits work? Call your insurance company and ask them the following questions:

Do I have out-of-network benefits for behavioral health?

Do I require pre-authorization to see a provider who is out-of-network?

What is my out-of-network deductible?

What amount will I be reimbursed for the following CPT Codes: 90791, 96136, 96137, 96130, 96131?

Is there a limit to how many 96137 and 96131 units I will be reimbursed for?
